4. NOTARIALS/AUTHENTICATION
5. DUAL CITIZENSHIP APPLICATION
6. REPORTS OF BIRTH/MARRIAGE
For PASSPORT AND DUAL CITIZENSHIP APPLICANTS, please mail your application
forms and supporting documents to the Consulate before the scheduled
Milwaukee Consular Outreach for pre-processing of your applications. You
are however required to appear at the venue to complete the following
requirements: 1) personal appearance; 2) signature and thumb mark; 3)
payment of fees
For OVERSEAS ABSENTEE VOTER REGISTRANTS, please bring a copy of your
passport or your birth certificate
